Escalating antimicrobial resistance among Enterobacteriaceae: focus on carbapenemases

چکیده

Introduction: Over the past few decades, antimicrobial resistance (AMR) has skyrocketed globally among bacteria within Family Enterobacteriaceae (i.e. Enterobacter spp, Klebsiella Escherichia coli, Proteus Serratia marcescens, Citrobacter and others). are intestinal flora important pathogens in nosocomial community settings. spread easily between humans may acquire AMR via plasmids or other mobile elements. The emergence of multidrug resistant (MDR) clones have greatly limited therapeutic options. Some infections untreatable with existing antimicrobials.Areas covered: authors discuss escalation CRE globally, epidemiology outcomes infections, optimal therapy, potential role several new antimicrobials to combat MDR organisms. An exhaustive search for literature related was performed using PubMed, following key words: resistance; carbapenemases; Enterobacterales; Enterobacteriaceae; pneumoniae; coli; global epidemiology; metallo-?-lactamases; New Delhi Metalloproteinase-1 (NDM-1); plasmidsExpert opinion: Innovation development classes antibacterial agents critical expand effective encourage judicious use antibiotics aggressive infection-control measures essential minimize AMR.

Rapid evolution and spread of carbapenemases among Enterobacteriaceae in Europe.

Plasmid-acquired carbapenemases in Enterobacteriaceae, which were first discovered in Europe in the 1990s, are now increasingly being identified at an alarming rate. Although their hydrolysis spectrum may vary, they hydrolyse most β-lactams, including carbapenems. They are mostly of the KPC, VIM, NDM and OXA-48 types. Their prevalence in Europe as reported in 2011 varies significantly from high...

Antimicrobial resistance in Enterobacteriaceae in Brazil: focus on β-lactams and polymyxins

During the last 30 years there has been a dissemination of plasmid-mediated β-lactamases in Enterobacteriaceae in Brazil. Extended spectrum β-lactamases (ESBL) are widely disseminated in the hospital setting and are detected in a lower frequency in the community setting. Cefotaximases are the most frequently detected ESBL type and Klebsiella pneumoniae is the predominant species among ESBL prod...

Should Canada focus on antimicrobial resistance?

The Canadian Institutes for Health Research’s new global health research guru wants Ottawa to put antimicrobial resistance on its list of foreign-aid research priorities. Speaking in Vancouver on Nov. 15 at Health Systems Research 2016, a biennial conference that attracted 2000 delegates from 101 countries, Steven Hoffman, scientific director of CIHR’s Institute of Population and Public Health,...

Carbapenemases in Enterobacteriaceae: types and molecular epidemiology.

The most important mechanism of carbapenem resistance in Enterobacteriaceae is the production of carbapenemases, although resistance can also result from the synergistic activity between AmpC-type or (to a lesser extent) extended-spectrum beta-lactamases combined with decreased outer membrane permeability. Three major molecular classes of carbapenemases are recognized: A, B and D. Classes A and...

Laboratory detection of Enterobacteriaceae that produce carbapenemases.

A study was designed to evaluate the modified Hodge test (MHT), Mastdiscs ID inhibitor combination disks (MDI), Rosco Diagnostica Neo-Sensitabs (RDS), metallo-β-lactamase (MBL) Etest, and in-house multiplex PCR for the detection of well-characterized carbapenemase-producing Enterobacteriaceae. One hundred forty-two nonrepeat clinical isolates of carbapenemase-producing Enterobacteriaceae (inclu...
